Auditing Multi-Agent Systems: What Decision-Makers Need to Understand Now

As organizations begin to deploy AI agents that act, decide, and collaborate autonomously, a new question emerges at the board level: how do we audit systems that are no longer static, but adaptive and distributed? Traditional IT audit approaches still apply, but they need to be extended to address the specific risks, behaviors, and human responsibilities within multi-agent systems.


What's happening

Many organizations are moving from isolated AI use cases to systems where multiple AI agents interact with each other and with existing IT infrastructure. These agents may handle customer communication, automate internal workflows, or support decision-making processes. Unlike traditional software, they can adapt their behavior based on context, learn from interactions, and influence each other's actions.

At the same time, humans should not be fully out of the loop. Employees review outputs, override decisions, set objectives, or intervene in edge cases. This creates hybrid systems where human judgment and machine behavior are intertwined. Decisions are no longer purely automated, but neither are they fully controlled by humans in a traditional sense.


Why this matters

For board members and executives, this shift has direct implications for governance, risk, and compliance. Established internal control systems rely on traceability, clear responsibility, and reproducibility. Multi-agent systems challenge all three, especially when human interaction is part of the system design.

From a risk perspective, errors or unintended behaviors can propagate across agents and be reinforced or overlooked by humans. There is also a risk of over-reliance, where human reviewers trust system outputs without sufficient scrutiny. From a compliance standpoint, accountability becomes more complex. It is not enough to ask which system produced a decision. You must also ask who validated it, who could have intervened, and under what conditions.

From a governance perspective, the presence of a "human in the loop" is often seen as a safeguard. In practice, however, it can create a false sense of control if roles, expectations, and limitations are not clearly defined.

If you are already familiar with IT audits and internal control frameworks, you have a strong foundation. Many principles remain valid. Segregation of duties, access controls, logging, monitoring, and change management are still essential.

However, multi-agent systems require you to extend these principles to include human interaction as an integral part of the system. Humans are no longer just users of the system. They are part of the control logic.

This means audit processes must address questions such as: When is human intervention required? What information is available to support human decisions? Are humans able to effectively challenge system outputs, or are they merely confirming them? And how is human behavior monitored and supported over time?

For organizations, this requires a shift from viewing human oversight as a simple control step to treating it as a component that must be designed, tested, and audited like any other part of the system.


What to do next

Start with the fundamentals and extend them where needed.

Ensure clear documentation, versioning, and access control for each agent. Logging should capture not only system actions, but also human interventions such as approvals, overrides, and ignored alerts.

Define explicitly where human involvement is required and what is expected. A "human in the loop" is only effective if roles, authority, and decision criteria are clear.

Strengthen observability. Monitor how agents interact, but also how humans respond. Patterns such as frequent overrides or blind acceptance of outputs are important signals.

Finally, ensure accountability remains clear. Even in distributed systems, ownership for decisions, oversight, and outcomes must be defined and visible at board level.
